Coos County Family Health Services has part-time and full-time positions available for an RN, LPN or CMA to work in our Berlin and Gorham, NH offices.

The staff nurse shall assist the medical provider in the provision of comprehensive primary care services. Responsible for managing, tracking, collaborating and coordinating patient health care needs including: medical, behavioral, oral, emotional, etc. with an emphasis on the whole patient.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Organizes and completes documentation, tracking, referrals, etc. in a timely manner.

Keeps flow of clinic structured and as on schedule as much as possible.

May schedule appointments and triage client calls.

Counsels clients on anatomy and physiology, health maintenance and other health care issues.

Performs on-site laboratory work and phlebotomy (if trained) for off-site laboratory work as well as blood pressures, weights and other routine procedures.

Maintains and records applicable client information in electronic medical records in an orderly fashion and according to the guidelines. Obtains and assesses client’s medical history and family history.

Assists medical provider with procedures and serves as a chaperone.

Maintains clinical tracking for client services: follow-up, acute care, chronic disease, preventive, lab, x-ray, referral, etc.

Actively participates in the development, implementation and evaluation of each client’s plan of care.

Sets up clinic rooms and organizes flow of clinic. Cleans laboratory/exam rooms and instruments after client visit.

Replenishes supplies and orders medical supplies as needed.

Dispenses non-prescriptive medication/contraceptives per pharmacy guidelines.

Dispenses prescriptive medication/contraceptives if RN. If an LPN, an RN or Medical Provider must be on site per pharmacy guidelines.

Attends meetings, workshops, and seminars and keeps current on primary care issues for all lifecycles.

May assist in agency planning, projects, and procedures.

Follows agency, State, and Federal policies, procedures, and guidelines, including (but not limited to) clinical, health, and safety, pharmacy, laboratory and nursing guidelines. Works within the scope of practice of an RN or LPN, as appropriate, and within agency guidelines.

Performs other job-related tasks as assigned, as time allows.
