RN/LPN/CMA
Multiple Clinical Office Nursing Positions available to work 20-36 hours per week at all three medical office locations. Some evening and weekend shifts may be available (shift stipend/incentive for weekends).
Part Time and Full Time Benefits Available.
Triage Support Nurse (LPN/RN)
Partial duties include E-prescribing, answering phones, printing and/or faxing medication refills as ordered by the provider and/or standing orders. Check prescription line for medication refill requests. Screen patient’s COVID – 19 needs via telephone. Consult with medical providers in order to recommend most appropriate action for the patient. Make necessary referrals for insurance authorizations. Document in the electronic medical record.
This position is 40 hours per week Monday – Friday and eligible for full-time benefits.

Communication Coordinator
Responsible for screening and directing telephone calls to teams; transferring calls to triage, clinical staff, or other departments; and assisting with team coordinator duties as time allows. Distributes and processes incoming and outgoing mail.
Must be available to work some evenings and weekends as applicable.

Operations and Practice Manager

Experience in Managing a medical and/or dental practice; knowledge of Community Health Centers and Grant requirements; experience in developing new policies/protocols; experience supervising a diverse team.
This position is Salaried/Exempt 40 hours per week with full time benefits.
Community Health Worker
32 - 40 hour per week position available Helping individuals, families, groups and communities develop their capacity and access to resources, including health insurance, food, housing, quality care and health information.
A high school degree or equivalent is required, along with a strong work ethic, good verbal and written communication skills, and willingness to learn.
RN/LPN/CMA – Per Diem
Per Diem opportunity for our Medical Practice in Colebrook. RN/LPN license or CMA Certification required. Experience working in an outpatient clinic a plus.

Part-Time Maintenance Technician – Per Diem
Evaluate maintenance requests, set priorities, and complete work assignments in a timely manner. Resolve facility repairs and maintenance issues promptly and cost effectively. Perform all maintenance work to properly maintain CCFHS facility and grounds. Perform routine inspections of all systems and equipment.
