36-40 Hours per week.

SUMMARY
Evaluate, writes, monitors, and coordinates behavior management plans with the clinical team in support of and consistent with direct patient care.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Ensures service quality and participates in care coordination to ensure proper communication between medical providers, patients and referral sources.

Direct Service Advocate - Berlin, NH
Coös County Family Health Services (CCFHS) is seeking a Direct Services Advocate to join our RESPONSE team in the Berlin office. This is a 24-hour per week, benefit-eligible position.
Position Summary
The Direct Service Advocate provides trauma-informed, survivor-centered support to individuals impacted by domestic and sexual violence and stalking. Support includes crisis intervention, emotional support, safety planning, court and medical advocacy, and connection to resources.
Essential Duties and Responsibilities
The following reflect core duties; other duties may be assigned as needed.
Direct Advocacy Services
• Provide in-person and phone-based advocacy for individuals impacted by domestic and sexual violence and stalking
• Offer crisis intervention, emotional support, safety planning, peer support, and referrals
• Facilitate and/or support survivor support groups, as assigned
• Accompany clients to hospitals, police departments, Child Advocacy Centers (CACs), and court proceedings as needed
• Assist with shelter intake and follow-up support when appropriate
• Maintain accurate, timely, and appropriate data entries in the client services database
On-Call Coverage
• Serve as scheduled staff backup on the 24/7 support line rotation, including nights and weekends
• Participate in other on-call shifts as assigned, such as weekday daytime coverage
• Provide support to on-call volunteers during shifts as needed
Community Collaboration & Communication
• Build collaborative relationships with local partners such as law enforcement, healthcare, legal, and social services
• Participate in local community meetings, multidisciplinary team (MDT) meetings, and case reviews as assigned
• Participate in outreach and awareness events to promote services and community engagement
• Attend regular staff meetings, supervision, and staff development opportunities
• Communicate effectively with team members regarding coverage, client needs, and service delivery
Minimum Qualifications
• Associate degree in human services, criminal justice, psychology, or a related field; or a minimum of two years of relevant experience
• Completion of the 30-hour Response Training Program (required upon hire) to ensure understanding of confidentiality, privilege, and program standards
• Maintain a minimum of 6 hours of annual training relevant to advocacy and trauma-informed care

Team Coordinator- Gorham Office
SUMMARY
Responsible for greeting patients; screening and directing telephone calls, inquiries, and enrolling patients for services; updating patient demographics, insurance information, consent for services, personal contacts, HIPAA information, etc; posting payments, co-pays and other related tasks as assigned; and working with clinical teams to address patient services and satisfaction.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Greets and assists patients in completing necessary forms
Helps maintain patient flow
Assists team with patient services: schedules appointments, arranges for patient call backs, transfers calls to triage as appropriate, prioritizes patient requests, maintains communication with teams, attends huddles, and assists with a variety of tasks
Relays accurate information to team coordinators: patient name, date of birth, phone number and reason for call
Prints clinical summary for patient at end of each office visit
Enrolls patients and assigns primary care provider if patient does not have specific request for a provider
Re-schedules patients when necessary.
Ensures demographic information is correct for all patients at each encounter.
Processes co-pays, payments and other related tasks
Updates and verifies insurance information, demographics, consent for services and disclosures, personal contacts (for emergencies), and other regulatory requires such as HIPAA.
Offers information about sliding fee
Ensures paperwork for release/request of information, legal documents, DCYF, workers compensation and other required paperwork is delivered to the appropriate medical records department or provider
Assists Communication Coordinator to answer calls as needed
Monitors flags and email for updates and responds as needed
Works as a team player
Follows agency’s policies, procedures and guidelines
Performs other job-related tasks as assigned

Position Summary
We are seeking a compassionate and detail-oriented Clinical Support Staff member (Medical Assistant, LPN, or RN) to join our mission-driven team. Key Responsibilities
Clinical Support

Prepare patients for visits, including obtaining vital signs, medical history, and health screenings.

Assist providers with examinations, procedures, and treatments within scope of practice.

Administer immunizations and medications per licensure and standing orders.

Collect and process laboratory specimens and perform point-of-care testing.

Maintain exam rooms, clinical equipment, and supplies in a clean and organized manner.
Care Coordination & Documentation

Accurately enter patient data, visit notes, and orders in the electronic health record (EHR) following organizational and regulatory standards.

Assist with care coordination, patient follow-up, and outreach for preventive and chronic care needs.

Provide patient education regarding medications, treatments, and self-care.

Team Collaboration & Quality Improvement

Qualifications
Required:

Current certification/licensure in New Hampshire:

MA: Medical Assistant Certification or equivalent experience preferred

Current CPR/BLS certification.

Strong computer and technology skills, including proficiency with electronic health records, Microsoft Office, and basic troubleshooting.

Staff Nurse RN- Colebrook, NH
SUMMARY
The staff nurse shall assist the medical provider in the provision of comprehensive primary care services. Responsible for managing, tracking, collaborating and coordinating patient health care needs including: medical, behavioral, oral, emotional, etc. with an emphasis on the whole patient.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Organizes and completes documentation, tracking, referrals, etc. in a timely manner.
Keeps flow of clinic structured and as on schedule as much as possible.
May schedule appointments and triage client calls.
Counsels clients on anatomy and physiology, health maintenance and other health care issues.
Performs on-site laboratory work and phlebotomy (if trained) for off-site laboratory work as well as blood pressures, weights and other routine procedures.
Maintains and records applicable client information in electronic medical records in an orderly fashion and according to the guidelines. Obtains and assesses client’s medical history and family history.
Assists medical provider with procedures and serves as a chaperone.
Maintains clinical tracking for client services: follow-up, acute care, chronic disease, preventive, lab, x-ray, referral, etc.
Actively participates in the development, implementation and evaluation of each client’s plan of care.
Sets up clinic rooms and organizes flow of clinic. Cleans laboratory/exam rooms and instruments after client visit.
Replenishes supplies and orders medical supplies as needed.
Dispenses non-prescriptive medication/contraceptives per pharmacy guidelines.
Dispenses prescriptive medication/contraceptives if RN. If an LPN, an RN or Medical Provider must be on site per pharmacy guidelines.
Attends meetings, workshops, and seminars and keeps current on primary care issues for all lifecycles.
May assist in agency planning, projects, and procedures.
Follows agency, State, and Federal policies, procedures, and guidelines, including (but not limited to) clinical, health, and safety, pharmacy, laboratory and nursing guidelines. Works within the scope of practice of an RN or LPN, as appropriate, and within agency guidelines.
Performs other job-related tasks as assigned, as time allows.
