Weeks Medical Center
Whitefield Clinic
Residents will complete their continuity clinic experience at the Weeks Medical Center Whitefield Clinic, one of several outpatient practice sites operated by Weeks Medical Center. Located in the heart of Whitefield, this modern clinic serves patients from across the North Country and provides a welcoming, team-based environment for resident education.
The continuity clinic is designed to provide comprehensive family medicine training across the lifespan. Residents will care for pediatric, adult, and geriatric patients while developing long-term relationships with their patient panel. The site also supports ambulatory pediatric training, allowing residents to gain additional experience in preventive care, developmental screening, and the management of common pediatric conditions.
Working alongside family medicine faculty and an interdisciplinary care team, residents will gain increasing autonomy in preventive care, chronic disease management, population health, and care coordination. This longitudinal experience prepares graduates to confidently practice full-scope family medicine and provide high-quality, patient-centered care in rural communities.
Weeks Medical Center
Weeks Medical Center (WMC) is the primary hospital partner for the residency program and serves as the site for Inpatient Family Medicine training. WMC is a 25-bed critical access hospital within the North Country Healthcare system, providing medical, surgical, and 24/7 emergency care to northern New Hampshire and northeastern Vermont.
With a history dating back to 1913, when the region’s first hospital was established in Lancaster, WMC has served the North Country for more than a century. The current hospital was opened in 1947 as Beatrice D. Weeks Memorial Hospital and has grown over the decades to meet the evolving healthcare needs of the community while maintaining the personalized care and close-knit relationships characteristic of rural medicine.
Today, WMC provides residents with broad inpatient and emergency medicine experiences in a community-focused setting dedicated to high-quality, patient-centered care. In addition to its hospital services, WMC operates six patient care centers throughout the region, extending access to primary and specialty care across the North Country.
Androscoggin Valley Hospital
Androscoggin Valley Hospital (AVH) is a key clinical training partner within the White Mountains Medical Education Consortium and a member of North Country Healthcare. As a critical access hospital serving northern New Hampshire, AVH provides residents with valuable rural healthcare experience through rotations in Emergency Medicine, General Surgery, Musculoskeletal and Sports Medicine, Cardiology, Pain Management, ICU and other specialty areas.
Residents benefit from working alongside experienced physicians and healthcare professionals in a community-focused setting that emphasizes high-quality, patient-centered care. AVH provides a diverse and robust learning environment for family medicine training.
Coos County Family Health Services
Gorham Clinic
The Coos County Family Health Services (CCFHS) Gorham Clinic is one of CCFHS’s multiple sites that provides comprehensive outpatient care, including primary care, pediatric, and behavioral health services. Within the residency program, the Gorham Clinic serves as a dedicated site for ambulatory pediatric training under the supervision of board-certified pediatric faculty.
Residents gain hands-on experience in the evaluation and management of acute and chronic pediatric conditions, preventive care, immunizations, developmental screening, and adolescent health.
Pleasant Street Clinic
Coos County Family Health Services’ Pleasant Street Clinic is an ambulatory training site within the White Mountains Medical Education Consortium. As a Federally Qualified Health Center, the clinic provides residents with valuable experience caring for diverse and underserved patient populations while addressing the unique healthcare needs of rural communities.
Residents may participate in rotations focused on Community Health, Health Systems Management, Population Health, Women’s Health, and other aspects of outpatient family medicine.
Children’s Hospital of Michigan – Detroit Medical Center
Children’s Hospital of Michigan (CHM) is a 226-bed academic children’s hospital serving the Detroit metropolitan area since 1886 and has a long-standing tradition of excellence in pediatric care.
Residents will complete a comprehensive inpatient pediatric rotation with close supervision and strong educational support. This experience provides exposure to a wide range of acute and complex pediatric conditions, strengthening clinical decision-making and inpatient management skills in a high-volume academic setting.
The rotation is designed to prepare residents to confidently care for pediatric patients they will encounter in both rural and community-based practice settings. To support this educational experience, the residency program provides housing and travel accommodations for residents during their rotation in Detroit.

Littleton Regional Healthcare
Littleton Regional Healthcare is a critical access hospital serving Coös and Grafton Counties in northern New Hampshire. As a key regional healthcare provider, it offers a broad range of inpatient and outpatient services and plays an essential role in supporting rural healthcare delivery across the North Country.
Within the residency program, Littleton Regional Healthcare serves as a core clinical site for Obstetrics training, providing residents with hands-on experience in prenatal care, labor and delivery, and postpartum management under close supervision. This rotation ensures robust exposure to maternal and prenatal care in a rural hospital setting, preparing residents to manage obstetric care in both community and resource-limited environments.

Coos County Nursing Home – Berlin
Coos County Nursing Home (CCNH) is a county-operated nursing facility in Berlin, New Hampshire, providing long-term care, short-term rehabilitation, and skilled nursing services for residents of Coös County.
Within the residency program, CCNH serves as a core clinical site for Long-Term Care and Geriatrics rotations during PGY-1 and PGY-2. Residents gain experience in the management of complex chronic disease, functional decline, dementia care, and care transitions, while working closely with an interdisciplinary team to support patients in a skilled nursing setting.
North Country Home Health and Hospice Agency
North Country Home Health and Hospice Agency (NCHHHA) is a member of North Country Healthcare and a partner within the White Mountains Medical Education Consortium. Serving 51 communities across Coös and Grafton Counties since 1970, NCHHHA provides comprehensive home health, hospice, and palliative care services throughout northern New Hampshire.
Within the residency program, NCHHHA serves as a key clinical training site for Home Health, Hospice, and Palliative Care rotations. Residents gain experience in delivering patient-centered care in the home setting, managing complex chronic illness, and supporting patients and families through serious illness and end-of-life care in rural communities.
