The Consortium is comprised of two regional healthcare providers, North Country Healthcare and Coös County Family Health Services.  North Country Healthcare is a 3-hospital system of acute care hospitals while Coös County Family Health Services provides primary care as the sole Federally Qualified Health Center in the region.  These entities have a strong history of collaboration to support comprehensive patient care to the region.

The Consortium is dedicated to providing healthcare delivery that prioritizes the needs, values, and engagement of the local community in both clinical and non-clinical aspects of care. This approach aligns with the residency mission of providing accessible, comprehensive, and culturally competent care to underserved populations.

The residency program is dedicated to:

  • Active outreach and partnerships with community organizations, schools, faith-based institutions, and local leaders to identify and address community health needs.
  • Services are tailored to reflect the cultural, linguistic, and social contexts of the populations served.
  • Use of community health workers (CHWs) or patient navigators from within the community to build trust and support care coordination.
  • Emphasis on integrating primary care with behavioral health, dental, substance use disorder treatment, and enabling services (e.g., transportation, case management).
  • Care teams work collaboratively to address social determinants of health, such as housing, food insecurity, and employment.
  • Focus on reducing health disparities through preventive care, health education, and chronic disease management tailored to the population.
  • Regularly conducted to guide service delivery, identify gaps, and develop programs that directly respond to community-defined priorities.
  • Encourages patients and community members to be advocates for their own health and for broader policy changes affecting their wellbeing.
  • Provides education and tools to build health literacy and support self-management of conditions.

Our approach to community-centered care goes beyond providing medical treatment—it embeds the program center in the fabric of the community, empowering residents, reducing barriers to care, and promoting health equity through responsive, inclusive, and holistic services.
