Coos County Family Health Services 

Pleasant Street Clinic

Coos County Family Health Services, Pleasant Street is the Primary Clinical Site and the Family Medicine Practice site. Residents will have rotations for their Introduction to Family Medicine, Continuity Clinic, Community Health, Health Systems Management, Population Health, and Gynecology-Women’s Health rotations. The patient care team creates a positive patient care experience by clearly defining roles and responsibilities, coordinating duties, setting patient care goals through communications and teamwork.

 

Gorham Clinic

The CCFHS, Gorham clinic offers primary care, pediatric, behavioral health, and podiatry services. The Gorham clinic will be used for resident training in Ambulatory Pediatrics under the direction of a board-certified pediatrician.

 

Androscoggin Valley Hospital

Androscoggin Valley Hospital (AVH) is the primary hospital partner and will serve as the site for Inpatient Family Medicine, Emergency Medicine, Obstetrics, General Surgery, Musculoskeletal and Sports Medicine, Cardiology, and Pain Management rotations. AVH is a critical access hospital that is a part of North Country Healthcare, a partner-member of the White Mountains Medical Education Consortium. It is rated best in the nation in patient satisfaction for Orthopaedic Surgery and Sports Medicine, Neurology, ENT, Pain Management, and Nursing.

 

Weeks Medical Center

Weeks Medical Center is another critical access hospital within the North Country Healthcare system. Their multidisciplinary treatment center is where residents will complete their Behavioral Health/SUD-OUD rotations. Behavioral health services here include individual and group mental health counseling, and EMDR (eye movement

desensitization and reprocessing). There is also treatment for individuals with substance use disorders using medication-assisted treatment and other methods

 

North Country Home Health and Hospice Agency

North Country Home Health and Hospice Agency (NCHHHA) is another facility that is part of North Country Healthcare, a partner-member of the White Mountains Medical Education Consortium that is clinical site for Hospice and Palliative/Home Health rotations.   NCHHHA has provided 51 communities in Coos and Grafton Counties in New

Hampshire with home health, hospice, palliative care, and other services since 1970. .

 

Littleton Regional Healthcare

Littleton Regional Healthcare is a critical access hospital in northern New Hampshire providing a variety of patient care services to Coos and Grafton Counties since 1907. The hospital is the major provider of maternal and infant care in the region and serves as a clinical site for the OB rotation.

 

Coos County Nursing Home – Berlin

The Coos County Nursing Home (CCNH) is a county facility that provides skilled nursing, short- and long-term nursing care, and rehabilitation services to the residents of Coos County. Residents will rotate through Coos County Nursing Home (CCNH) for their Long-Term Care/Geriatrics rotation during PGY-1 and PGY-2.

 

Children’s Hospital of Michigan – Detroit Medical Center

Children’s Hospital of Michigan (CHM) is a 226-bed academic children’s hospital serving the Detroit metro area since 1886.  CHM is well-known for their work in pediatric medicine, surgery, and research.  Residents will complete a robust and challenging inpatient pediatric experience in a large, urban university setting that provides a variety of pathology to prepare them for any scenario they may encounter during their career.
